Client Forms

Please be sure to complete all client intake forms prior to arriving for your scheduled massage session. Doing so will help your massage therapist to better plan for your session and maximize your treatment time.

All client forms must be completed prior to any treatment being provided. Refusal to complete the required client forms will result in the cancellation of your session.

Arrival & Pre-session Protocol

Arriving by the start of your scheduled appointment time allows for you to receive optimal services from your massage therapist.

You will spend the beginning of your session discussing important information related to your treatment with your massage therapist. During this conversation, it is highly encouraged that you ask questions and voice any concerns or updates you have regarding your treatment. Once the conversation has concluded, the massage therapist will step away to allow you time to undress to the level of your comfort and accommodate yourself underneath the drape on the massage table.

If needed, please use the restroom prior to your session to ensure your comfort during the treatment session.

Medical Conditions & Health Information

It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued.

The client must also keep the massage therapist informed of any changes in health conditions.

Massage may lead to adverse reactions in certain situations or with certain medical conditions. The massage therapist will evaluate your health history as communicated on your in-take form and ask you questions to ensure it is safe for you to receive massage.

In the event the massage therapist is uncertain that massage will be of benefit to you, they may ask you to provide a note from your physician stating that it is safe for you to receive massage services.

Please provide complete details of medical conditions and medications to your massage therapist during the intake process.


For clients undergoing chemo and radiation therapies: Please note that we require a doctor’s note that states the doctor is aware of and agrees to the desired treatment.

Failure to inform the massage therapist of all medical conditions and medications may place you at increased risk for adverse reactions. Massage Tucson, LLC and Bruno Loya are not responsible for any adverse reactions to receiving massage therapy services if the client fails to disclose all relevant information regarding the client's health history, medical conditions, current diagnoses, past or present injuries, prescribed medications, non-prescription medications, recreational substances, or other relevant information. By proceeding with massage therapy services with Massage Tucson, LLC, you acknowledge that you will not withhold any information from your massage therapist that is relevant to the service(s) to be provided.

Scope of Practice

Massage Tucson, LLC is operated by Bruno Loya (MT-28334) who is a licensed massage professional in the state of Arizona.

Massage Therapy is a profession in which the practitioner applies manual techniques, and may apply adjunctive therapies, with the intention of positively affecting the health and well-being of the client.

Licensed Massage Therapists do not diagnose or prescribe for medical conditions. Your massage therapist may refer you to a medical doctor for diagnosis or treatment. Our services are not intended as an alternative for proper medical attention for specific conditions. Please refer to a medical doctor for any specific condition which requires medical treatment.

Massage Tucson, LLC provides therapeutic massage services to help clients experience healing, stress relief, pain management, general wellness, improved functionality, increased range of motion and flexibility, and general self-care. Sensual, erotic, or sexual massage, body rubs, or manual stimulation is not within the scope of the therapeutic massage practice as regulated by the Arizona State Board of Massage Therapy.

Informed Consent

Prior to each massage session, the treatment plan will be discussed with you. You will be asked to sign the treatment consent form stating that you have read the information, understand it, and agree to comply with the professional massage therapy policies and procedures during your initial visit. Clients who we have not seen for at least a year may also be asked to update this form.

Client Rights & Privacy

The client has a right to prompt and professional service in an environment that is clean, private, and safe.

The client may choose to: leave on as much clothing as needed for comfort, refuse any massage methods, stop massage at any time for any reason.

Massage Tucson, LLC respects the client's right to privacy. We treat all client visits and conditions as confidential. As such, we will not discuss your visit with anybody outside of our staff without your expressed written permission.

A court of law may order the client’s healthcare records released to the court as part of a legal proceeding.

Massage therapists are obligated to report information about the abuse of a child, elderly person, or mentally or physically challenged person. Massage therapists are also obligated to report plans or threats of self-harm or harm to another person to legal authorities.




Right of Refusal

Massage Tucson, LLC retains the right of refusal at any time. In certain situations, Massage Tucson, LLC will refuse to accept someone as a client and/or once the person is a client Massage Tucson, LLC may refuse the client a return visit.

Circumstances for refusal include but are not limited to: when a client is rude, loud, inappropriate, solicits the massage therapist for work outside of their scope of practice, and other conditions not listed that Massage Tucson, LLC see as unfit for a professional client relationship.

Payment Policies

Payment is due in full at the time of service. Acceptable methods of payment include cash, Zelle, PayPal, and CashApp. Massage Tucson, LLC will begin accepting credit card payments soon. Payment by check is not accepted.

By proceeding with your appointment and receiving massage services from Massage Tucson, LLC you are agreeing to pay the total cost of the service(s) you've received. Non-payment will result in forfeiture of any future business with Massage Tucson, LLC until the unpaid balance is settled. Any client with a non-payment history will be required to pre-pay for any future services.

Cancellation Policy

Your therapy times are reserved exclusively for you. We value your business and ask that you respect scheduling policies. We also understand that unforeseen circumstances do occur. Should you need to cancel or reschedule, please notify us at least 24 hours in advance.

Cancellations within 24-hours of the scheduled appointment time are responsible for paying 50% of the total cost for service(s) booked. Cancellations within 3-hours of the scheduled appointment time are responsible for paying the total cost for service(s) booked.

Note: If you are sick or feeling unwell prior to your appointment, please contact your massage therapist and request to reschedule your appointment for a later date. We appreciate you respecting your health and ours by not proceeding with your treatment session while you are ill.

No-show & Late Arrival

No-show clients will be charged the full cost of the scheduled service(s). The full cost of no-show bookings must be paid in-full before any future service(s) may be provided.

Clients who arrive late to their session may be asked to reschedule. Clients arriving late will still be charged the full cost for the service(s) booked, and the session will end promptly at the originally scheduled time.

Refund Policy

Due to the nature of services provided, no refunds will be provided for massage services rendered.
